Jack and co have blown open the hatch. the find an old riusted ladder broken off halway down. They return to the beach. Jack makes a speech while Locke gears up to investigate the hatch. Kate goes with him. Shannon can't find Vincent. Sayid tries to help her. She sees Walt in the jungle. Hurley tells Jack sbout the numbers. Locke lowers Kate down the hole, she screams and all is silent. Locke follows her in
In the flashback we see Jack having to choose between two patients from the same car accident. He saves the girl who will become his bride but beieves she is paralyzed. While running the steps in a stadium to relieve stress, he meets Desmond who makes him feel better. Jack returns to the hospital to find that he was wrong, he had repaired the damage and the girl will not be paralyzed.
Jack goes to the hatch but there's no sign of Locke or Kate. He lowers himself inside to find a bunker and computer. Locke speaks and Jack realizes he has a gun to his head. The gunman steps out and it's Desmond, the guy from the stadium.
