We open with a Juliet flashback. We see she is a doctor giving an injection to a cancer patient.
Jack sabotages Ben's surgery and refuses to help unless the Others let Kate and Sawyer go. He gets on the Walkie-Talkie and tells SKate to Run. They beat up Pickett and take off. Juliet says Jack is bluffing and tells The others to go get SKate back.
In Juliet's flashback, she is stealing medication from her Job. Her boss is her ex and she almost gets caught, but he is too busy gettin hot n heavy with his date to question her too much.
Juliet and Tom tell Jack about the second island. Jack yells about Juliet wanting him to kill Ben. SKate is on the beach looking for a way off the island when the walkie talkie gets shot. Alex saves SKate from Pickett and Co. Ben asks for Juliet.
In the flashback Juliet's Ex tells her he knows what shes doing (stealing meds/testing) to help her sister and he wants in on it.
Tom tells Juliet Ben wants to see her. Alex tells SKate she has a boat they can use but wants their help rescuing her boyfriend Karl first. Jack gives Juliet 3 minutes with Ben. She comes out and tells him to finish the surgery, that she will help SKate escape.
In the flashback we see a doctor trying to recruit Juliet, but she is afraid her ex will stand in her way.
Alex and co, take out a guard and go to rescue Karl who is being brainwashed. Picket arrives soon after. Juliet tells him Ben said to let SKate go.
In the flashback, Juliet is telling her sister (the cancer patient) about the interview. Hert sister tells her the medication worked, and she's pregnant. Juliet goes to tell her ex. He gets hit by a bus and dies.
Jack talks to Tom while he continues operating on Ben. Alex and SKate make it to the boat. Picket tries to stop them so Juliet shoots him. She lets Karl and SKate go, but not Alex. Kate walkie-talkies Jack while he finishes the surgery. They leave in the boat.
In the flashback, we see the interview guy asking Juliet to join them now that her ex can no longer prevent it.
Juliet tells Jack that she has been on the island three years and that Ben told her if she got Jack to finish the surgery, he'd let her go home.
